What is HCS?

Home and Community-Based Services (HCS) is a Medicaid-funded program that provides support and services to individuals with intellectual and developmental disabilities, enabling them to live in their own homes or communities instead of institutional settings. HCS focuses on person-centered care, empowering individuals to make choices and decisions about their own lives.

We offer a range of Home and Community-Based Services (HCS) to help individuals live, grow, and thrive within their communities, including:

HCS Services

The HCS provider can also offer Personal Assistance Services, habilitation, or emergency response services through the Community First Choice (CFC) program for individuals residing in their own or family homes. Additionally, individuals can receive unlimited prescriptions through the Medicaid Vendor Drug Program.

Community First Choice (CFC)

The Community First Choice (CFC) program provides services to individuals living in their own home or with their family. Additionally, they can receive unlimited prescriptions through the Medicaid Vendor Drug Program.

The Consumer Directed Services

The Consumer Directed Services (CDS) option is available for this program, allowing individuals living in their own or family home to self-direct certain services and take on responsibilities within the program.
